Wessex AHSN have edited their web page, produced a learning report and video which outline the lessons-learnt, useful resources and learning from the TCAM work
The TCAM work, previously led by Wessex AHSN and supported by the AHSN Network is now a nationally-commissioned service called the NHS Discharge Medicines Service (DMS). Their updated website page can be viewed below:
