PharmOutcomes now available to use for Asthma Referrals to GP surgery
For any patient identified as needing a Personalised Asthma Action Plan (adult) or a spacer (child), a referral to their GP surgery can now be made via PharmOutcomes.
The referral service can be accessed by selecting the service called ‘Quality criterion – Asthma referrals’ listed down the left hand side of the screen.
When you access the service both options for Action Plan and Spacer will be showing, but once you enter the patient’s date of birth the spacer question will be hidden if the patient is 16 years or over.
