TCAM launch event for Andover, Basingstoke & Winchester Hospitals

Over the next few weeks representatives from the CPSC (Myself and Debby), Hampshire Hospital Team & Wessex AHSN will visit 88 pharmacies in and immediately around Andover, Basingstoke and Winchester to talk to the managers, pharmacists and teams about the forthcoming new TCAM (Transfer of Care Around Medicines) initiative.

TCAM will be launched at this evening event and will go live shortly after. The key thing is that we want to successfully contact, meet & confirm as many of you as possible to attend this.

Our ambition is for patient follow-up’s to occur in as many local pharmacies for as many of the hospital referred patients generated by TCAM as possible.

Following the event we hope that every pharmacy will be able to check their PharmOutcomes screen for referrals being sent and then positively report whether they have seen the patient or not. Doing this well will see an increase in numbers (and possibly quality) of MURs & NMS delivered at pharmacies involved, therefore improving the care given to their patients coming out of hospital as well as generating vital income for individual businesses

CPSC hope that we achieve good results by demonstrating a high % of persons being seen by us. The net effect should be better understanding of changes to medication for patients, improved health prospects and a significant decrease in hospital readmission rates for those people seen in pharmacies

There is no direct additional remuneration for the confirmation completion on PharmOutcomes following the electronic referral of an individuals discharge notes from hospital to their chosen pharmacy. The confirmation required by the pharmacy however is very quick and details just whether the patient was seen or not

The remuneration that a pharmacy receives will be generated by the subsequent provision of either an NMS or post discharge MUR service (NHS England funded), as all patients referred will be suitable for one of these. This should therefore help to drive up NMS provision across your pharmacies, as well as help more pharmacies achieve the 400 cap of MURs than currently do
