'Transfer of Care Around Medicines (TCAM) service' - launching soon from Hampshire Hospitals

The team have recently decided to have a bespoke engagement event to launch the TCAM service, share information and answer questions. Details are yet to be finally confirmed, but we will let you know ASAP (likely to be around 3rd week of October)

Hampshire Hospitals (HHFT) will soon be able to share an individual's patient discharge summary information directly with their nominated community pharmacy using the PharmOutcomes digital platform.
Details of the launch will be shared by the HHFT team attending a number of our pre-arranged Pharmacy events, so that you can chat with them during your dinner, prior to the start of the formal part of each evenings agenda.
Please make sure you attend the bespoke event (when announced) one of the events listed in the communication briefing below to hear first hand about the TCAM roll-out.
