Request for TCAM feedback, good news stories & useful anecdotes

The TCAM service in Southampton between community pharmacies and UHS is now well established. The stakeholder steering group (which includes CPSC) that originally planned and help steer progress toward implementation and launch are now wanting to produce an update on progress so far. The update aims to share both qualitative and quantitative information for all stakeholders to see.

Your help is therefore being asked for to provide examples of some of the practical ways in which the service has helped people being discharged from hospital and then receiving a follow-up conversation with you (including an MUR or NMS, other service or not).
