MOTIVE service goes live on Isle of Wight

MOTIVE is now live! St Mary's hospital are now able to share an individual's patient discharge summary information directly with their nominated community pharmacy using the PharmOutcomes digital platform.

We recommend that pharmacies print this out and use it for initial training and then future reference with their team. It's useful to have to hand over the first few months until the service becomes more familiar to the team and actions required get bedded in.
We are hoping that all pharmacies will wish to 'Accept' and then 'Complete' the referrals on PharmOutcomes so that we get some quality data evidence regarding the usefulness and potential value of the service, as well as the savings identified through reduced hospital admissions.
