Hampshire County Council - Weight Management Service

For Hampshire County Council area only - excludes Southampton, Portsmouth and Isle of Wight.

As of the 1st October, Hampshire County Council has a new Tier 2 Weight Management Service. Shape Up 4 Life is a free 12 week programme that patients can self refer to via the website: To be eligible patients need to live in Hampshire County Council area or be registered at a GP surgery within the county area.

The programme will provide both nutritional advice and exercise via face-to-face meetings, virtual meetings and support with an app. The programme is tailored to the patient's needs
(men only, BME, LD, mental health LGBT-Q , pre and post-natal women, women only).
