Your help in completing this survey will be much appreciated

Please find below a message from the Lead Endocrine and Diabetes Pharmacist, Hampshire Hospital NHS Trust who are now starting to re-start their project on DOAC counselling following instigation of this treatment.

Dear Community Pharmacists

We are seeking your perspective about your involvement in counselling patients newly started on anticoagulation medication and/or discharged from hospital for an anticoagulation related issue.  Although there have been huge changes over the recent months, it would be helpful if you could reflect on your practice prior to COVID-19  Any comments in relation to the current situation can be recorded in the final question.  We are hoping that you can take the time to undertake a short (maximum 10 minute) survey which can be accessed via the link below:

This would enable us to understand what counselling you are currently able to offer and how we can help and support you with new resources and hospital referrals, generating new counselling opportunities in the future.  This has the potential to impact upon patient care and empower patients to better manage their medicines thereby reducing the incidence of bleeding related side effects.

Thank you in advance for completing this survey which should take a maximum of 10 minutes to complete.

This survey is part of a collaborative project (funded by a Pfizer MEGS Grant) between Wessex Academic Health Science Network and Hampshire Hospitals NHS Foundation Trust [HHFT] and Community Pharmacy South Central to improve counselling, advice and support for patients prescribed anticoagulation medicines.
