The process asks all NHS community pharmacy contractors to complete a short screening questionnaire consisting of 10 active questions which should be answered.

NHS England - South (Wessex) issued the full CPAF process details directly to all community pharmacy contractors in the following attached letter below from Karen Denham. The letter contains links to an advanced view of the screening questionnaire, a copy of the full CPAF to view and details of the date that NHSBSA will provide instructions on how to access & complete the questionnaire.
NHS England have also produced a CPAF Process document that clearly shows the timeline dates for activity required, which we recommend you use to chart your progress towards successful submission.
