UPDATE: New Best Practice guide for Southampton Smoking Referral Service

A handy reference document for the pharmacy team to have to hand when referring people contemplating quitting
Its now around six months since the Southampton Healthy Living (SHL) Incentivised Referral Scheme for community pharmacies was commissioned. The volume of suitable patient referrals per month to the service is growing slowly in some provider locations, but remains very low from other providers.
A recent review of the service has identified that to encourage pharmacies to increase the rate of referral, it was important to share best practices as well as increase access to the information and promotional material.

A4 Best Practice Guide - please print off and ensure the pharmacy team understand the simple PharmOutcomes referral process, then keep this to hand for reference when discussing smoking cessation with your customers.
