  • Patient stories from hospital and community
  • Best practice sharing from UHS team and high activity TCAM providing pharmacies
  • Update on data - how are we doing, what difference is this making, potential NHS outcomes realised so far?
  • How is it for you ? Please come prepared to share your experiences
  • Next steps and an update on Hampshire Hospitals

The meeting is coordinated and organised by Wessex AHSN / UHS / CPSC and is being financially supported by NHS England - South (Wessex). James Allen (UHS) and Richard Buxton (CPSC) will be presenting with contributions and support from other representatives attending who will be able to take questions and feedback during the evening.

It's important for pharmacists from both in and around Southampton to book-in and attend this follow-up event to ensure we all provide a fantastic experience to the many discharged patients using this service, as well as maximising the benefits from working collaboratively across our different healthcare sectors.
