Transfer of Care Around Medicines (TCAM) - Southampton service celebration 'From Good to Great'

THIS EVENT HAS BEEN CANCELLED - Around 1000 patients have now been referred using TCAM at UHS since September 2017. Come and join us at this ideal milestone event to assess how things are going and to share best practices & behaviours amongst all professionals.
TCAM - Enabling closer working between the hospital discharge team and community pharmacies
The meeting will provide:
The story so far - an update from James Allen, UHS - including a 'what you said, what we did' session and a recap of procedures & PharmOutcomes process (useful if you missed the initial launch event)**Community view - feedback from Richard Buxton, CPSC - 'what we've heard'Patient stories from hospital and communityBest practice sharing from UHS team and high activity TCAM providing pharmaciesUpdate on data - how are we doing, what difference is this making, potential NHS outcomes realised so far?How is it for you ? Please come prepared to share your experiencesNext steps and an update on Hampshire Hospitals
The meeting is coordinated and organised by Wessex AHSN / UHS / CPSC and is being financially supported by NHS England - South (Wessex). James Allen (UHS) and Richard Buxton (CPSC) will be presenting with contributions and support from other representatives attending who will be able to take questions and feedback during the evening.
