SURVEY MONKEY REQUEST: Use of 'Diabetes Hypoglycaemia Support Pack' resource in community pharmacies

We're conducting a very short survey for pharmacies within the West Hampshire CCG only. Your participation would be much appreciated. Please read on for details...
A valuable new hypoglycaemia resource pack for pharmacists, pharmacy teams and people with diabetes is being delivered to every West Hampshire community pharmacy.
We want to be able to evaluate the packs usefulness to the role of community pharmacists, so we have prepared a short 10 question Survey Monkey which is estimated to take only 3 minutes.
We would like community pharmacies within the West Hampshire CCG area to complete this survey now and then complete another one in mid October (after attending one of the two planned development evenings) so that we can understand any significant changes.
