Complete the declaration as soon as possible
In accordance with the Pharmaceutical Regulations NHSE are collating the details of pharmacy provision during the Christmas and New Year holiday period to ensure adequate coverage across the area. Please complete the online declaration via the link below as soon as possible.
If you plan to be closed on all the bank holidays you still need to complete the declaration.

Changes to Supplementary or Core Hours
Contractors may notify NHSE of a change to their supplementary or core opening hours under their terms of service as set out in the Regulations.
If you wish to notify a temporary change, please complete the relevant form.
