Please attach these printable information notes to prescriptions of people with diabetes
Diabetes UK have asked if it is possible to add the following patient information note to medicines of people presenting prescriptions that have diabetes.
The information can be printed as A4 and then cut down to produce 4 patient notes per sheet.
